- Absence Form (as needed). Please review the absence policy/form in the Handbook. We need every student at every rehearsal so that our team can function! Should something unexpected arise, please follow those procedures for approval.
Digital Forms Checklist: Due online by 7/31/24!
____ Student Food/T-Shirt Survey - We need this ASAP to plan meals and order show shirts!
____ Time and Talent Survey - Please fill out one for each parent!
____ Band Camp Volunteer Form: If you have a spare hour or two, we could use your help!
____ Join the “BAND“ for quick in-app updates! Make sure your notification settings are turned on!
____ 2024 Band Camp Schedule: Check out our plans! Details/adjustments posted daily in BAND.
____ Spirit Wear Order - For any students, family members, or friends! - Students will receive one show shirt and one “Friday“ shirt, included in their band fees. They will need a pair of solid black shorts for our warmer-weather events. These can be purchased here!

____ UCPS Request for Health Information. We will turn these in to the MRHS Nurse.
____ UCPS Medication Consent Form. We will turn these in to the MRHS Nurse.
____ UCPS Authorization for Self-Carry (as needed). We will turn these in to the MRHS Nurse.
____ UCPS Instrument Agreement Form (Low Brass/Low Reeds)
____ Marching Mavericks Travel Release Form - Covers ALL travel for the season.
____ Signed Uniform Agreement (Instrumental or Color Guard).
