- Hard Copy Forms Checklist:
____ UCPS Request for Health Information. We will turn this in to the MRHS Nurse. Every student MUST complete one of these!
____ UCPS Medication Consent Form. We will turn these in to the MRHS Nurse. This form is only needed IF your student has a prescription medication that needs to be taken during the day or for our overnight competition. PLEASE complete one form for EACH medication.
____ UCPS Authorization for Self-Carry (as needed). We will turn these in to the MRHS Nurse. This form is only needed IF your student has a prescription medication such as an inhaler for asthma, epi pen for allergies, that their physician has determined they are allowed to administer on their own.
____ UCPS Instrument Agreement Form (Low Brass/Low Reeds)
____ Marching Mavericks Travel Release Form - Covers ALL travel for the season.
____ Signed Uniform Agreement (Instrumental or Color Guard).
